The inTransition Program is a voluntary program to support servicemembers, veterans, guard, and reserve of all services as they move between health care systems or providers.
The inTransition program is designed to make sure the servicemembers are given the necessary resources to maintain treatment, connect with a provider and achieve positive outcomes. Servicemembers from all branches and veterans with any category of discharge qualify to use the inTransition program. Information about local community resources, support groups, healthy lifestyle options, benefit enrollment and other health resources are also available.
