We understand the caregiving journey is often stressful and overwhelming.

It can be difficult to find time to focus on yourself. The Elizabeth Dole Foundation and Yoga4Caregivers have teamed up to provide education and empowerment to the military caregiver community through the power of yoga. Join us in exploring self-care and mindfulness through the practice of yoga, while connecting with others for support. Together, we can empower each other to rest, recharge, and reconnect with our inner resilience.

Informed Consent and Liability Waiver Release for Participation in the Online Yoga Program hosted by the Elizabeth Dole Foundation in partnership with the Caregiver Wellness Collective, Inc.

I UNDERSTAND THAT BY READING THIS AGREEMENT, I AM ENTERING INTO A BINDING CONTRACT, AND WILL BE BOUND BY THE PROVISIONS SET FORTH BELOW.

In consideration of the services of the Elizabeth Dole Foundation in partnership with the Caregiver Wellness Collective, Inc., including, but not limited to its agents, owners, officers, instructors, subcontractors, volunteers, participants, employees, affiliates, and all other persons or entities acting in any capacity on its behalf, I HEREBY ACKNOWLEDGE AND AGREE TO IRREVOCABLY RELEASE AND WAIVE, INDEMNIFY, AND DISCHARGE THE ELIZABETH DOLE FOUNDATION AND THE CAREGIVER WELLNESS COLLECTIVE, INC. OF ANY AND ALL PAST, PRESENT, OR FUTURE CLAIMS ON BEHALF OF MYSELF, MY CHILDREN, MY PARENTS, MY HEIRS, ASSIGNS, PERSONAL REPRESENTATIVES/AGENTS, AND ESTATE, in my agreement and consent of the following:

I UNDERSTAND that I am voluntarily participating in the Online Yoga Program hosted by the Elizabeth Dole Foundation in partnership with the Caregiver Wellness Collective, Inc.

I ACKNOWLEDGE that the Online Yoga Program is offered online by remote transmission and that your participation will not be monitored by an instructor, and therefore no individual guidance will be provided. You agree to monitor your own health and capabilities. You further agree that should any portion of the instruction threaten or compromise your health that you will cease participation.

I UNDERSTAND that yoga includes physical movements, as well as an opportunity for relaxation, stress re-education, and relief of muscular tension. As is the case with any physical activity, THE RISK OF INJURY TO ME OR TO THIRD PARTIES, EVEN SERIOUS OR DISABLING, IS ALWAYS PRESENT AND CANNOT BE ENTIRELY ELIMINATED.

I UNDERSTAND that it is my responsibility to consult with a physician prior to and regarding my participation in this program. I REPRESENT AND WARRANT THAT I HAVE NO MEDICAL CONDITION preventing my participation in the program.

I ASSUME FULL RESPONSIBILITY for any risks, injuries, or damage known or unknown which I might incur as a result of participating in the program. Such injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness, including death. I knowingly, voluntarily, and expressly WAIVE ANY CLAIM I may have against the Elizabeth Dole Foundation and the Caregiver Wellness Collective, Inc. and/or their designated agents, owners, officers, instructors, subcontractors, volunteers, participants, employees, affiliates, and all other persons or entities acting in any capacity on its behalf for injury or damages that I may sustain as a result of participating in the program.

I, my heirs or representatives FOREVER RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE the Elizabeth Dole Foundation or the Caregiver Wellness Collective, Inc., their designated agents, owners, officers, instructors, subcontractors, volunteers, participants, employees, affiliates, and all other persons or entities acting in any capacity on its behalf for any injury, death, claim, demand, or cause of action of any kind resulting from or related to my participation in the programs caused by their negligence or other acts.

I RECOGNIZE that there is risk involved in the types of activities offered. Therefore, I ACCEPT FINANCIAL RESPONSIBILITY for any injury that I may cause either to myself or to any other participant due to my negligence. Should I, or anyone acting on my behalf, be required to incur attorney’s fees and costs to enforce this agreement, I agree to pay all such fees and costs.

I further AGREE to INDEMNIFY AND HOLD HARMLESS the Elizabeth Dole Foundation and the Caregiver Wellness Collective, Inc, their agents, owners, officers, instructors, subcontractors, volunteers, participants, employees, affiliates, and all other persons or entities acting in any capacity on its behalf from liability for the injury or death of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in the activities offered.

I ACKNOWLEDGE that the Online Yoga Program may be recorded by video, audio, and/or photo. I understand that these recordings may be made publicly available at some later date by the Elizabeth Dole Foundation or the Caregiver Wellness Collective, Inc. and their agents, employees or contractors and I give them my permission to use my image, voice and participation freely with no compensation to be given to me under any condition.

I have read the above informed consent and liability waiver and release liability and fully understand its contents. I am legally competent to voluntarily agree to the terms and conditions stated above.
