Before You Begin Your Application
Verification Documentation Required
Please have one of the following documents ready to upload:
- DD214
- Veteran Health Identification Card (VHIC)
- Uniformed Services Identification Card (Military ID)
- VA Benefits Award Letter
- VA Caregiver Program Award Letter
Financial Assistance Requests
If you are requesting financial assistance, please also upload any supporting documentation related to your request, such as:
- Statements
- Bills or invoices
- Estimates or quotes
Vendor Requirements
Please note that EDF pays vendors directly. If EDF is making a payment to a landlord, utility company, contractor, repair provider, medical provider, or other service vendor, a completed W-9 is required before payment can be processed. Obtaining the W-9 as early as possible can help prevent delays in processing approved assistance.
What is a W-9?
A W-9 is an IRS form used to collect a business’s or individual’s tax identification information, including their name, address, and Tax ID Number (EIN or SSN). EDF uses this information to process payments and comply with IRS reporting requirements.
During the Application Process
You will be asked to select your role from a drop-down menu. Please choose the option that best describes you:
- Caregiver
- Active-Duty Family Member
- Survivor
- Dole Caregiver Fellow
If you are a member of the Dole Caregiver Fellowship, please select “Dole Caregiver Fellow.”
Each role is equally important, and the application is designed to gather information specific to your unique experiences and needs. Selecting the role that best describes you helps us provide the most appropriate support, resources, and opportunities throughout the application process.
