PMCN is a coordinating entity with a network of collaborative partners providing services to better meet the needs of our region’s military and veteran community. We connect individuals and families to critical resources and services by fostering coordination, collaboration and partnerships among service providers; training community-based providers on military and veteran culture and the unique needs of our military, including caregivers; and conducting outreach to providers and our military and veteran communities to increase knowledge of and access to resources. We provide web-based and call-in information & assistance, and we provide navigation (case management) support for individuals and family caregivers who have multiple or complex needs, or who find the myriad of local services intimidating or overwhelming. We work with them one-on-one to obtain assistance from multiple agencies so that these individuals and families do not become frustrated and end up in further crisis.
